AUTHORITY: The undersigned represents that they have the authority to enter into this Agreement and that the terms of this Agreement are in accordance with the governance or mission of the recipient entity.

Recipient represents that they are primarily organized to provide law enforcement, fire and/or emergency medical services (paid or volunteer), an educational institution, a community based organization that provides community emergency preparedness and response training, or other educational outreach program to the Recipient’s community.

SCOPE OF AGREEMENT: No Provider of Alertar y Preparar LISTOS Program publication, pamphlet, flyer, card or any other printed, visual or audio media may be used for commercial purposes or sold. This includes charging a fee to class attendees or accepting program specific compensation for an individual Alertar y Preparar LISTOS Program class. For the purposes of this Agreement, the receipt of material donations from community sponsors that are directly used in support of a class such as photocopying, printing, class refreshments or other components of the Alertar y Preparar LISTOS Program items noted in the curricula is not considered commercial compensation. Such arrangements of cost recovery is supported and encouraged by Provider.

It is in the mutual interest of both Parties, and in the best interests of the community, that emergency preparedness education is engaging, tailored, and culturally-appropriate for maximum impact and retention. To achieve this, Provider will deliver Alertar y Preparar LISTOS Program instructor training and class educational materials to Recipient at no charge.

All publications provided by Provider are licensed for public and private use under Creative Commons attribution-non-commercial-no derivatives 4.0 International Public License.

No modifications to Provider copyrighted materials are authorized without written consent. For the purposes of this Agreement, modification does not include the addition of localized points of contact, telephone numbers, addresses and other specific content for the furtherance of the Program as covered by this Agreement.

For the purposes of monitoring the effectiveness and statistical validation of the program, Recipient agrees to deliver to Provider, upon request, copies of all class registration sign-up sheets for each daily class held by the Recipient.

Recipient agrees to comply with Title VI of the Civil Rights Act of 1964, Public Law §88-352, (42 USC §2000 et seq.), which provides that no person in the United States will, on the grounds of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to the discrimination under any program or activity receiving Federal financial assistance.

All information submitted with your application for registration is for internal use only. No information will be sold or otherwise provided to any third party except California Volunteers.
